Pathophysiology and Epidemiology

Palmoplantar psoriasis affects approximately 12–16% of individuals living with psoriasis, representing a substantial patient population in clinical practice. The condition arises from dysregulated immune responses involving T-cell activation and pro-inflammatory cytokines, particularly interleukin-17 and interleukin-23, which drive keratinocyte hyperproliferation and abnormal differentiation. When localised to the feet, the thick stratum corneum of the plantar skin contributes to the characteristic hyperkeratotic morphology and resistance to conventional therapies.

Palmoplantar pustulosis, historically considered a variant of palmoplantar psoriasis, presents with sterile intraepidermal pustules and may represent a distinct entity, though the classification remains debated in dermatological literature. Both conditions are chronic, relapsing disorders associated with significant impairment in quality of life due to pain, fissuring, and functional limitations during ambulation.

Clinical Presentation

Cutaneous Manifestations

Plantar psoriasis classically presents as bilateral, symmetrical, well-demarcated erythematous plaques with sharp borders extending across the soles. The overlying scale is typically thick and may exhibit a silvery appearance, though on darker skin tones the plaques may appear greyish or hyperpigmented. Deep fissuring is a hallmark feature, often causing severe pain and bleeding that significantly impairs mobility and weight-bearing activities.

In palmoplantar pustulosis, the initial presentation involves crops of small, sterile pustules that evolve from yellow to brown before becoming scaly and desquamating. Nail Involvement

Histopathological examination reveals acanthosis, parakeratosis, neutrophilic infiltration of the stratum corneum (Munro microabscesses), and dilated capillaries in the dermal papillae.

Diagnostic Approach

Diagnosis is primarily clinical, based on the characteristic appearance, distribution, and patient history. Key diagnostic features include symmetrical involvement, well-demarcated plaques, nail changes, and personal or family history of psoriasis. When fungal infection is suspected, skin scrapings for microscopy and culture should be obtained to exclude tinea.

For patients with suspected psoriatic arthritis, referral to rheumatology is warranted for comprehensive assessment including inflammatory markers, imaging (radiography or musculoskeletal ultrasound), and consideration of disease-modifying therapy. Podiatrists play a crucial role in early recognition of enthesitis and dactylitis, facilitating timely multidisciplinary management.

Management Strategies

Topical Therapies

First-line treatment for localised plantar psoriasis includes high-potency topical corticosteroids, often under occlusion to enhance penetration through the thick plantar stratum corneum. Intermittent rather than continuous use is recommended to minimise tachyphylaxis and adverse effects. Adjunctive therapies include emollients to reduce scaling and fissuring, keratolytic agents such as urea (20–40%) or salicylic acid to soften hyperkeratosis, and coal tar preparations for anti-inflammatory and antiproliferative effects.

Topical calcipotriene (vitamin D analogue) may be used in combination with corticosteroids for enhanced efficacy, though penetration through thick plantar plaques remains challenging. For palmoplantar pustulosis, ultrapotent topical steroids are typically employed intermittently, as continuous treatment may aggravate the condition.

Phototherapy

Narrowband ultraviolet B (NB-UVB) phototherapy and psoralen plus UVA (PUVA) are effective options for widespread or recalcitrant disease. However, the thickness of plantar skin may limit light penetration, requiring higher doses or longer treatment courses.

Systemic Therapies

For moderate-to-severe disease or significant functional impairment, systemic agents are indicated. Traditional options include methotrexate, cyclosporine, and acitretin, which modulate immune responses and keratinocyte proliferation. These require careful monitoring for hepatotoxicity, nephrotoxicity, and teratogenicity.

Biologic therapies targeting tumour necrosis factor-alpha (TNF-α), interleukin-17, and interleukin-23 have demonstrated promising results for palmoplantar psoriasis, with significant improvements in plaque clearance and quality of life. Agents such as etanercept, infliximab, secukinumab, and ixekizumab are increasingly employed for refractory cases.

Podiatric and Mechanical Management

Podiatric intervention is essential for addressing biomechanical complications and optimising function. Custom orthotic devices can redistribute plantar pressures, reduce stress on inflamed entheses, and accommodate structural deformities. Supportive footwear with cushioned soles and adequate depth accommodates swollen digits and reduces shear forces.

For enthesitis-related pain, conservative measures including rest, ice, activity modification, and non-steroidal anti-inflammatory drugs provide symptomatic relief. Physical therapy focusing on stretching and strengthening exercises maintains joint mobility and muscle function. In cases of severe deformity or refractory pain, surgical interventions such as arthrodesis or joint reconstruction may be considered in coordination with orthopaedic colleagues

Multidisciplinary Care

Optimal management requires collaboration between podiatrists, dermatologists, and rheumatologists. While systemic disease is managed with disease-modifying antirheumatic drugs (DMARDs) and biologics by rheumatology, podiatrists focus on mechanical protection, footwear optimisation, wound care for fissures, and monitoring for secondary complications. Regular follow-up enables early detection of disease progression and adjustment of therapeutic strategies.

Prognosis and Quality of Life

Palmoplantar psoriasis is a chronic, relapsing condition with no definitive cure, though modern biologic therapies have improved long-term outcomes. The functional disability associated with painful fissures, altered gait, and footwear restrictions substantially impacts quality of life, employment, and psychological well-being. Early diagnosis, aggressive treatment, and comprehensive podiatric care are essential to minimise morbidity and maintain mobility.

Psoriasis affecting the feet encompasses a spectrum of cutaneous and articular manifestations requiring a nuanced, multidisciplinary approach. Recognition of characteristic clinical features, appropriate diagnostic evaluation, and implementation of evidence-based therapies—from topical agents to biologics and mechanical interventions—enable podiatrists to significantly improve patient outcomes. Ongoing research into targeted immunomodulatory therapies continues to expand the therapeutic armamentarium, offering hope for enhanced disease control and quality of life for affected individuals.
